What Is VO2 Max Testing?
VO2 max, or maximal oxygen uptake, refers to the maximum rate of oxygen your body can utilize during intense exercise, measured in milliliters of oxygen per kilogram of body weight per minute (ml/kg/min). According to the University of Virginia School of Medicine, VO2 max testing is considered the gold standard for assessing aerobic and cardiovascular fitness.1
V=volume
O2 = oxygen
Max = maximum
VO2 Max = ml/kg/min
During a VO2 max test, you exercise at progressively increasing intensities on a treadmill, stationary bike, or rowing ergometer while wearing a specialized metabolic analyzer. At Biohackr Health, we use a treadmill, with the ability to run or walk, changing speed and incline depending on your ability. The VO2 machine measures the oxygen you consume and carbon dioxide you produce, allowing us to calculate your precise VO2 max value, determine when you shift from burning fat to carbohydrates, and identify your aerobic and anaerobic thresholds.
The physical test typically lasts 10-15 minutes of active exercise, with the entire appointment taking approximately 45-60 minutes to include pre-testing setup, the assessment itself, and post-testing analysis and recommendations.

Why VO2 Max Testing Matters for Your Health
Cardiovascular Health and Longevity
According to Harvard Health, a high VO2 max correlates to better physical fitness and is associated with a lower risk for cardiovascular disease and increased longevity. Research demonstrates that individuals with higher VO2 max values have significantly better long-term health outcomes, making this metric one of the most important indicators of overall cardiovascular health.2
Performance Optimization
Understanding your VO2 max allows you to train more effectively. Rather than guessing at appropriate exercise intensities, you’ll know exactly which heart rate zones correspond to your aerobic base, fat vs. carbohydrate utilization, lactate threshold, and maximum effort capacity. This precision enables you to:
- Design training programs tailored to your specific fitness level
- Identify your optimal fat-burning and endurance zones
- Track improvements over time with objective data
- Prevent overtraining and reduce injury risk
- Maximize the efficiency of every workout
Preventive Health Insights
VO2 max testing provides early warning signs of cardiovascular decline.3 As we age, VO2 max naturally decreases, but the rate of decline can be slowed significantly through targeted exercise and lifestyle interventions. Regular testing allows you to monitor this critical metric and adjust your health strategy proactively.
The Benefits of VO2 Max Testing
Accurate Fitness Assessment
VO2 max testing eliminates guesswork from fitness evaluation. Unlike wearable devices that estimate your aerobic capacity, laboratory-based testing provides precise, validated measurements of your actual individual cardiovascular fitness. This accuracy is essential for creating effective training programs and tracking meaningful progress.
Personalized Training Zones
One of the most valuable outcomes of VO2 max testing is the identification of your personal heart rate training zones. These zones correspond to different physiological states:
- Recovery Zone (50-60% of VO2 max): Light activity for recovery and warm ups.
- Aerobic Zone (60-70%): Moderate intensity exercise that builds endurance and aerobic capacity, primarily utilizing fat.
- Tempo Zone (70-80%): Higher intensity work that improves lactate threshold
- Threshold Zone (80-90%): High-intensity effort at the edge of sustainable effort to improve speed and power.
- Maximum Zone (90-100% of VO2 max): All-out effort at your VO2 max. This is used for short bursts of high intensity training, crucial to develop peak performance.
With these zones precisely defined for you specifically, you can tailor training to target specific goals- fat-burning, speed, peak performance- rather than exercising at random intensities.
Progress Tracking and Accountability
Regular VO2 max testing provides objective evidence of fitness improvements. Whether you’re training for an athletic event, recovering from illness, or pursuing general health optimization, retesting every 3-4 months gives you concrete data showing whether your training strategy is working.
Integration with Regenerative Medicine Protocols
At Biohackr Health, VO2 max testing is uniquely integrated with our regenerative medicine and anti-aging therapies. Understanding your cardiovascular capacity allows us to optimize treatments like NAD+ therapy, creatine, peptide protocols, and bioidentical hormone replacement to support your fitness goals and overall health optimization.
Muscle mass and mitochondrial efficiency in making ATP (the energy used in every cell in your body) are key factors in energy production. Building muscle, creatine, and NAD+ are all keys to improving cardiovascular fitness. None of this works though without oxygen.
What to Expect During Your VO2 Max Test
Before Your Test
You need to make an appointment for this test, as proper preparation ensures accurate results. If you are not a current patient at Biohackr Health, please call and schedule a free consultation to discuss the VO2 max test, which can be done in person or on the phone. Who Benefits from VO2 Max Testing?
Competitive and Recreational Athletes
Athletes at all levels benefit from precise VO2 max data. Whether you’re training for a marathon, triathlon, cycling event, or team sport, understanding your aerobic capacity and training zones optimizes performance and reduces injury risk. We also have an extended VO2 max test for endurance athletes that lets you see real time your energy consumption, which can help guide fueling and pace.
Fitness Enthusiasts and Health-Conscious Individuals
If you’re committed to fitness and want to train smarter, VO2 max testing provides the objective data needed to structure effective workouts and track progress toward your goals. Is your goal fat burning? Improving endurance? Improving peak performance? Training for those goals involves different zones. Your smartwatch or ring can give you estimates, but this is the gold standard and shows your actual results. Estimates from wearables can be off by as much as 30%.
Individuals Pursuing Longevity and Health Optimization
VO2 max is one of the strongest predictors of long-term health and lifespan. If you’re focused on optimizing your health span and preventing age-related decline, regular VO2 max testing is an essential component of your health strategy.

Those Recovering from Illness or Injury
VO2 max testing provides objective benchmarks for recovery progress. Whether you’re returning to activity after surgery, illness, or injury, testing helps ensure you’re progressing safely and effectively.
For those who need a surgery that may affect lung or heart function, this also lets you see your baseline, changes after surgery, and recovery.
Individuals Undergoing Regenerative Medicine Treatments
At Biohackr Health, VO2 max testing is particularly valuable for those pursuing regenerative medicine therapies. Testing before and after treatment protocols allows us to measure the impact of therapies like NAD+ infusions, creatine, weight changes, peptide protocols, and hormone optimization on your cardiovascular fitness and aerobic capacity.

What will I see on my test results?
- An estimate of your cardiovascular fitness. This incorporates your age, height, weight, and metrics from your VO2 test.
- VO2 max. This is when you change from aerobic (oxygen) to anaerobic activity. Your workout threshold is soon after this.
- Fat vs. carbohydrate utilization. For most, when you get to a higher heart rate your body burns carbohydrates for energy.
- VE/VO2 ratio. VE = minute ventilation, which is the total volume of air inhaled or exhaled per minute. This looks at how fast you breathe and how much air you bring in, which is key to see how healthy your lungs are, and how efficient your air exchange (getting in oxygen and getting rid of carbon dioxide) is.
- Respiratory Exchange Rate. RER – VCO2/V02. This shows when you are burning fat vs. carbohydrates. When RER =1, that is close to your VO2 max.

Biohackr Health offers physician-supervised VO2 max testing at our Palo Alto location, with San Francisco coming soon. Our unique approach combines precision medical grade cardiovascular fitness assessment with regenerative medicine expertise, allowing us to optimize your testing results within the context of comprehensive health optimization. Whether you're in San Francisco, Palo Alto, or the surrounding Bay Area, our team is ready to help you understand your cardiovascular capacity and develop a personalized strategy for fitness and longevity.
How often should I retest my VO2 Max?
For most individuals, retesting every 12 weeks provides meaningful data on training progress and fitness improvements. Athletes in competitive training may benefit from more frequent testing, while those focused on general health optimization may retest 2-3 times per year. Our team will recommend an appropriate testing schedule based on your goals and current fitness level.
Can you improve your VO2 max?
Yes. This is a key to what we advocate at Biohackr Health. The best way to improve VO2 max varies from person to person. You can increase VO2 max by improving heart performance, lung function, improving muscle mass, improving mitochondrial function (think ATP, NAD+, creatine), losing weight and fat, doing high intensity interval training, and increasing aerobic activity. Again, we recommend looking at the whole picture of your individual health when figuring out your plan.
What is VE?
VE is minute ventilation. It is the total volume of air inhaled or exhaled per minute, measured in liters/minute. VE = RR respiratory rate x TV tidal volume. Lungs and the air exchange in them are important to cardiovascular fitness. When doing VO2 max testing, the VE/VO2 ratio shows the heart and lung efficiency. The lower the VE/VO2 ratio, the more efficient you are during exercise - less ventilation is needed to get the same oxygen. This helps you identify safe and effective training zones.
What if I'm not an athlete? Is VO2 Max testing still valuable?
Yes! VO2 max is one of the strongest predictors of overall health and longevity, regardless of athletic aspirations. Even if you're not training for competition, understanding your cardiovascular fitness and tracking improvements over time is valuable for preventive health, motivation, and ensuring your exercise routine is effectively improving your fitness. VO2 max testing shows your changes in real time during the test- when you change from fat utilization to carbohydrates, your lung capacity, your heart rate change, and when you change from aerobic to anaerobic exercise. VO2 max testing is particularly valuable for individuals pursuing longevity and health optimization.
How does VO2 Max testing compare to fitness trackers and smartwatches?
While fitness trackers and smartwatches provide convenient VO2 max estimates, they use algorithms. These assumptions may not reflect your individual physiology, and can be off by 30%. Medical device VO2 max testing provides precise, clinical, validated measurements. Additionally, medical device testing provides detailed real time information about your aerobic and anaerobic thresholds, VE/VO2, RER, HR vs VO2, fat utilization vs carbohydrate utilization, lactate threshold, and personalized training zones. Wearable devices cannot determine these accurately. For serious athletes and those committed to health optimization, VO2 max medical device testing is the gold standard.
What is RER?
RER = Respiratory exchange ratio. It tells you when you go from aerobic to anaerobic activity and when you go from using fats for energy to carbohydrates. RER = VCO2/VO2. When RER = 1.0 you are close to your VO2 max, your maximal effort.
How are heart rate and VO2 max related?
There is a positive correlation between heart rate and VO2 max. As your heart rate goes up, your oxygen consumption also goes up. It is a great proxy for what level you are in your work out and your fitness level, but VO2 max testing is the gold standard. VO2 max testing on our medical CardiocoachPro machine measures your individual, actual oxygen and carbon dioxide levels to give you accurate, precise, real time results.
How does smoking or vaping affect VO2 max?
Chronic smoking or exposure to smoke narrows the small airways of your lung and reduces lung compliance. It also increases inflammation, mucus, and creates oxidative injury, making it harder to exchange gases (take in oxygen and release carbon dioxide). This lowers your oxygen uptake, which lowers VO2 max. It is dose dependent. The longer and more you smoke, the larger the decline in minute ventilation and lung function.
